我理解並同意以下內容，並願意根據以下條款和條件申請成為「非牟利獸醫服務協會」的義工:
By signing below, I understand and agree to the following and willingly apply to be volunteers of the Non-profit Veterinary Services Society (the "Society") under the following terms and conditions:

1 「非牟利獸醫服務協會」義工並不界定為「僱員」，故本人在進行義務工作期間，恕未能受到「僱員補償條例」的工傷保障。
Volunteers of the Society are not employees of the Society and are not covered by any work-related injury protection under the Employees' Compensation Ordinance when volunteering.

2 「非牟利獸醫服務協會」暫時未能為本人購買個人意外保險，亦不用承擔本人在進行義務工作時而引致意外、受傷的各項責任。
The Society will not purchase any personal accidents insurance for volunteers and will not be held liable for any accidents, injuries and/or any other responsibilities when conducting voluntary work.

3 由於「非牟利獸醫服務協會」之義務工作有機會接觸動物，故協會建議本人尋求醫生的專業意見，極力鼓勵按情況及需要自費作以下各項疫苗注射︰
Volunteers will come into contact with all types of animals. The Society recommends all volunteers to seek professional medical advice and strongly encourages the following vaccinations to be taken at volunteers' own expense:

3.1 破傷風疫苗
Tetanus vaccine
3.2 狂犬病疫苗
Rabies vaccine
3.3 有關疫苗 - 任何可以幫助及預防本人受到其它病菌或病毒感染的疫苗保護
Any other applicable vaccines that can help to protect volunteers from other germs and/or viruses.

（若閣下對疫苗注射有任何疑問，本會鼓勵尋求醫生建議自費安排，本會概不負責。）
(For any questions on vaccine, volunteers are encouraged to seek consultative advice from medical orders at their own expenses and the Society will not be responsible for any cost incurred as a result thereof.)

4 本人授權「非牟利獸醫服務協會」於本人一旦發生意外、受傷及發病時，為本人尋求緊急醫護治療，並聯絡本人於義工申請表上所填報的緊急事故聯絡人。
I authorize the Society to contact the emergency contact stated in this application form in case of emergencies, injuries, and illnesses which I may suffer during the course of volunteering and to seek immediate medical attention on my behalf at such event.

5 本人提供之資料均屬正確及真實，並無虛假。本人亦明白如年齡未滿十八歲，需得到本人之家長或監護人同意申請，及由家長或監護人簽署此免責聲明書。(請細閱家長同意部分)
All information provided in this form is stated to the best of my ability and are accurate and true. I understand that if I have not reached legal age of 18 at the time of signing this form, parental consent is required and this form must be signed by my parent(s) or legal guardian (please read the parental consent part of this form).

6 為保障社區動物不被滋擾，在所有計劃行動中的資料必須保密不對外公開，本人會嚴格遵守，亦清楚「非牟利獸醫服務協會」會保留一切追究權力。
To avoid nuisance for community animals, I understand that all information contained in action projects must remain confidential. I shall abide by such confidentiality and understand that the Society reserves all adhering legal rights.

7 本人明白不得私下利用「非牟利獸醫服務協會」或 "NPV" 之名義進行任何活動 (包括但不僅限於籌款), 亦清楚協會保留一切追究權力。
I understand that I cannot use the name of the Society or "NPV" to conduct any activities (including but not limited to fundraising), and that the Society reserves all adhering legal rights thereto.

8 如參與行動組別或小組,如因私人理由未能參與請跟組長聯絡；如半年內沒參與行動或小組，無故失約者將被除名而不另行通知。
If I, for any personal reason, cannot participate activities led by the action group or small groups, shall inform my group leader of my absence in advance. I understand that if I do not participate in any action group or small group activities for over half a year, or if I do not show up for service without giving prior notification, I may be removed from being a volunteer of the Society without any prior notice.

9 義工必須遵守一切「非牟利獸醫服務協會」所頒布的規則, 包括任何已知會和/或未知會而更新了的義工手冊和協會規則上所列明的條款。
All volunteers must abide by all rules and regulations enforced by the Society, which includes all announced and/or unannounced terms stated in the latest versions of volunteers' handbook and Society's handbook.

10 本人明白協會將保留所有接受或拒絕義工申請的權利, 而最終決定權亦歸於協會所有, 協會並保留隨時終止義工義務服務的權利。
I understand that the Society reserves the right to accept or reject any applications to volunteer, and the final decision rests with the Society who also retains the right to terminate any volunteers from volunteering.

我們將透過郵寄、電子郵件、電話、短信和傳真等方式發放 NPV 的活動、服務推廣和籌款活動資訊。 We will distribute NPV's activity information, service promotions, and fundraising activities through direct mailing, email, telephone, SMS, and fax.
